Lab 07: Integrating a Microsoft Forms form with SharePoint

Microsoft 365 user interface
Given the dynamic nature of Microsoft cloud tools, you might experience UI (User Interface) changes that occur after the development of this training content. As a result, the lab instructions and lab steps might not align correctly.
Cloud updates occur frequently, so you might encounter UI changes before this training content updates. If this occurs, adapt to the changes, and then work through them in the labs as needed.
Lab Scenario
In the ‘Raposa do Rio’ organization, all forms and surveys are made on paper. With digital transformation it is necessary to move all forms to a digital format.
Objectives
The organization has a Microsoft 365 subscription with Microsoft Forms and wanted to use this product to replace paper forms and surveys.
Instructions
Before you start
You must have access to a Microsoft 365 account with Microsoft Forms.
Sign in to the lab environment
Use the credentials provider by the instructor.
Exercise 1: Reusing an existing Internet form.
Microsoft Forms has templates that they can use and change to suit their needs.
The template you will use is an ‘English grammar test’ with a maximum score of 100 points. Microsoft Forms also supports a practical way for the learning process, offering a new way to review, test and reinforce knowledge. This mode is only available for quizzes.

View the type of existing questions

Enable practical mode.

Click ellipsis (…).

Choose Settings.

Enable Practice Mode.

Test the questions by clicking the Preview button.

Change the title to English grammar test (Example).

Return to Microsof Form and check if the form was saved automatically.
Exercise 2: Create a new form.
In this exercise you will create a form in the language of your choice with a maximum of 4 questions defined by you.

Create a new form with a name of your choice.

Set the primary language.

Add a maximum of 4 questions of your choice.

Get link to share and collect responses

Share the link on your team channel for this training on Microsoft Teams.
Exercise 3: Add the created form in SharePoint

Access SharePoint online.

Choose a SharePoint site that you created. If you haven’t created one, create a new Communication site and choose Blank template.

Edit the main page.

Add a new section with one column.

Hover your mouse above or below an existing web part and you’ll see a line with a circled +, like this:

Click + to see a list of web parts to choose from. Click the Microsoft Forms web part.

Select Add existing form.

If you want to share one of your own forms, go to
https://forms.office.com
and open the form you want to share on your SharePoint in Microsoft 365 page. -
Go back to your SharePoint in Microsoft 365 page. In the Form web address field, paste the web address of your form.

Make sure Collect responses is selected, then click OK to refresh the page and see your form.

Click Republish button.

Test the form.
